- What to bring to your DOT Physical
- Bring any required forms, including the DOT Medical Examination Report with "Driver Information" and "Health History" sections completed.
- Drivers License
- If you have any medical conditions you may be required to bring additional paperwork. Feel free to call us ahead of time if you have any questions to avoid any delay of your certification. See "Tips for Preparing for you DOT Physical" below to see if you need to bring any further information/documentation to your physical.

- Insulin Assessment
- If you are currently taking Insulin for your Diabetes have your prescribing physician fill out this form.
- Please bring the completed form with you to your DOT Physical appointment.
